Abstract

Aim. To compare serum levels of high-sensitivity C-reactive protein (CRP) and some other risk factors in women with premature coronary heart disease (PCHD) and their apparently healthy counterparts. Material and methods. We determined levels of CRP, total, HDL and LDL cholesterol (CH), triglycerides (TG), apoprotein B (apoB), parameters of haemostasis (plasminogen activator inhibitor, plasminogen, antithrombin III, antiplasmin, fibrinogen), plasma glucose and insulin before and after oral glucose load in 28 women aged 45-55 (mean 49.9) years with overt CHD and 28 age-matched women (mean age 48.7 yrs, p=0.122) without CHD (selected from a group of spouses of men withPCHD). Results. Groups of patients and controls had high proportion of menopausal women (64.3 and 50%, respectively, ns). Patients and controls had similar average levels of LDL CH (156+/-6.5 and 155+/-9.8 mg/dl, respectively) while patients had higher TG (195+/-18.4 vs 103+/-7.8 mg/dl, p=0.0001), apoB (148+/-10.2 vs 114+/-6.3 mg/dl, p=0.016), lower HDL CH (42+/-1.3 vs 53/-2.5 mg/dl, p=0.0029) and LDLCH/apoB ratio (1.12 vs 1.46, p=0.0007). Basal level of insulin was higher in patients (15.4+/-2.01 vs 9.9+/-1.72 muU/ml, p=0.031). There were no differences between groups in parameters of haemostasis (except PAI activity which was higher in patients [18.2+/-2.33vs 10,9+/-2.08 IU/ml, p=0.0091). Patients had higher body mass index (32.2+/-1,46vs 27.6+/-0.85, p=0.012) and waist to hip ratio (p=0.0002). CRP was significantly higher in patients compared with controls (2.66+/-0.86 and 0.59+/-0.21 mg/l, respectively, p=0.0224) and most closely related to body mass index (r-0.57, p<0.001) and waist circumference (r=0.517, p<0.001). However atmultifactorial analysis the only factor independently linked to PCHD was plasma TG level. Conclusion. Thus PCHD in women was associated with signs of"metabolic syndrome": inclination to abdominal obesity, higher TG and PAI activity, lower HDLCH and LDLCH/apo B ratio and higher level of CRP. However, plasma level of TG was the only parameter independently related to the presence of PCHD.
