Abstract

The highest suicide risk is found among the elderly and those suffering from mental illness and depressive pathologies, in particular. However, the risk related to the onset of pathological depression is less frequently recognized. As part of our work with the elderly in the Consultation Liaison Psychiatry (CLP) Service at the "Maggiore" General Hospital in Bologna, we decided to review the available data with the aim of identifying the current situation. Using the Medline, we collected all relevant publications over the last ten years together with their main references. In the field of elderly depression, we dedicated our attention to the differentiation and evaluation of the predictors of evolution and outcome, considering suicide itself as the worst outcome. Recent studies identify the onset-age of the depressive episode, medical comorbidity, cognitive impairment and lack of social support as important outcome predictors of depression. More specific considerations examined and discussed the onset-age as an outcome predictor, early- or late-onset depression of the elderly, treatment response and associated suicide risk. The suicide study and research on predictors led to the conclusion that differentiation of late-onset depression among the elderly isfeasible and worthwhile.
