Abstract

Neurologic erectile dysfunction (ED) can be defined broadly as an inability to sustain or maintain a penile erection due to neurologic impairment or dysfunction. There can be considerable overlap between neurologic ED and psychogenic, vasculogenic, and endocrinologic ED, depending on the condition. This can be due to a broad spectrum of unrelated diseases or circumstances, which result in peripheral neuropathies, nerve damage, or traumatic loss of neural function. Underlying etiologies can include diabetes, multiple sclerosis, Parkinson's disease, hereditary peripheral neuropathies, spina bifida, radical retropubic prostatectomy (RRP), spinal cord injury, and lumbardisc herniation; each with its own set of unique and confounding pathophysiologies. In this discourse following a brief review of neurophysiology, etiologies of disease, diagnosis and office work-up, and treatment of neurologic ED are discussed. Finally, prospectives for future research and treatment strategies are explored.
