Abstract

Hepatic encephalopathy (HE) is a complex neuropsychiatric disorder secondary to acute or chronic failure of the liver. No specific diagnostic markersfor this condition are available and the exact neuro-chemical mechanism underlying this condition is unknown. The neurotoxic effects of ammonia accumulation, the production of "false neuro-transmitters", and the enhancement of the inhibitory GABAergic tone in the CNS have been proposed as plausiblemechanisms for this disorders. Nevertheless, researches exploring these hypotheses have yielded contradictory findings. Several reports suggest that the increased levels of endogenous benzodiazepine receptor ligands (BZDRL) may play a role in the pathogenesis of HE, thought the evidence supporting this hypothesis is inconclusive. This suggests that neuropsychiatric symptoms in HE are difficult to explain only in terms of the accumulation of these compounds.
