Abstract

Estrogen-based replacement therapy is the recommended first-line approach to preventing osteoporosis in postmenopausal women, particularly those at increased risk for bone loss and development of fractures. The earlier treatment is started after menopause, and the longer it is continued, the greater the benefit in terms of maintaining bone mass and preventing fractures. There is recent evidence that estrogen therapy may also decrease the incidence of osteoarthritis, but not rheumatoid arthritis, in postmenopausal women. From the point of view of public health, education, screening, and therapeutic interventions are the most pressing needs associated with the prevention of osteoporosis.
