Abstract

In 1973 Rosenhan and seven of his colleagues simulated heaving voices in order to obtain admission to twelve psychiatric units. Each person received a diagnosis of schizophrenia or manic depression, in remission, although the symptoms disappeared immediately on admission and auditory hallucinationsalone are insufficient for a diagnosis of schizophrenia. By highlighting the failure To identify healthy subjects in the middle of a psychiatric service, Rosenhan wished to illustrate both the contextual nature of psychiatryand the inability to objectively validate a diagnosis. However, far from weakening the fortress of psychiatry. Rosenhan's paper and the ensuing Controversy provided the supporters of the DSM III with an unprecedented opportunity to obtain a decisive victory thus assuring the triumph of American psychiatry. The author reviews the issues raised in this debate in order to demonstrate how the question of context, as illustrated by the clinical significance attached to a single auditory hallucination, was appropriated and integrated into the DSM thereby establishing contemporary psychiatric policy.
