Abstract

ACTH secreting macroadenomas of the anterior pituitary gland are found in about 20% of patients with Gushing's disease [1]. These larger tumors appear to be more aggressive and resistant to conventional treatments than theirsmaller counterparts, ACTH secreting microadenomas, We believe that a heightened awareness of the difficulties encountered in the management of affected patients will enable treating physicians: 1) to select appropriate treatment regimens for patients with residual and recurrent hypercortisolism after initial surgery; 2) to design appropriate follow up plans to detect disease recurrences at the earliest possible time; and, 3) to provide sufficient education to their patients about the expectations of therapy and the natural history of the disease process. We will use a case study to emphasizesome of the more important nuances of these large tumors.
