Abstract

Patients after exstrophy of the bladder may present with multiple problems during pregnancy and childbirth. Pelvic floor anatomy may be changed. Frequent complications during pregnancy include prolapse and upper and lower urinary tract infections resulting in premature birth. Birth per vaginam may cause urinary or faecal incontinence. Cesarean section increases the risk of surgical injuries due to adhesions. Depending on previous operations recommendations for obstetric management differ. Some authors prefer Cesarean section for all patients, others recommend a Cesarean section only for patients with prolapse and previousureteric bower implantation. Some publications suggest Cesarean section only for fetal reasons. Overall we prefer an elective Cesaren section for patients who underwent a reconstructive operation after bladderexstrophy mainly to prevent incontinence. Furthermore, an emergency Cesaren section could be difficult with regard to the changed anatomical situation and should be avoided.
