Abstract

A series of 304 surgically treated women with endometrial carcinoma was studied. These comprised 46 patients with endometrioid carcinomas associated with a hyperplastic endometrium, 209 women with endometrioidcarcinomas associated with an atrophic endometrium and 49 patients with nonendometrioid carcinomas associated with an atrophic endometrium. Patients with nonendometrioid carcinomas had the worst survival rate while endometrioid carcinomas associated with an atrophic endometrium carried a worse prognosis than did those arising from a hyperplastic endometrium. The difference in prognosis for the two groups of endometrioid carcinomas was due largely to the greater proportion of G2 and G3neoplasms among those arising from an atrophic endometrium, there being no statistically significant difference in survival rate between G1carcinomas in the two groups. The demographic features of the two groups of patients with endometrioid carcinomas were similar but ii is suggested that the pathogenesis of the two types of endometrioid carcinoma may differ.
