Abstract

Cyclosporin A has been used successfully in adults with refractory dermatomyositis (DM) or polymyositis (PM), Cyclosporin A was given to six patients with juvenile dermatomyositis who either failed to respond to corticosteroid and immunosuppressive therapy or developed severe side effects of corticosteroid therapy. Clinical improvement allowing either a substantial reduction or discontinuation of corticosteroid therapy were seen in all six patients, Three patients had a relapse upon cyclosporin A discontinuation; all three responded to cyclosporin A andcorticosteroid therapy. Residual plasma cyclosporin levels were in the 51-247 ng/ml range. Adverse events recorded during the mean follow-up of 69.5 months were uncommon and mild. Cyclosporin A may be an effective adjunctive treatment in severe juvenile dermatomyositis when conventional therapy is ineffective or induces unacceptable side effects.
