Abstract

This multicenter randomized, double blind trial compared the clinicalefficacy and safety of cefotiam hexetil (CTM) 200 mg bid with those of amoxicillin/clavulanic acid (ACA) I g bid for 10 days, in adults with chronic otitis media. The aim of the trial was to compare rates of clinical response (cure) at the end of treatment. Rhinopharyngeal swabswere obtained at enrollment. 190 adults (95 in each group), referred by 43 ENT specialists, were enrolled and treated between October 1994 and September 1995. 103 men and 87 women were enrolled with a mean (m /- sd) age of 49 +/- 17 years. Population for both regimens was comparable at inclusion according to demographic and clinical data. In the intent to treat analysis, end of treatment efficacy was demonstrated by: cure for 61 % (57/94) in the CTM group, and 65 % (61/94) of the ACAgroup (p = 0.55). Relapse was observed at follow-up on D30 for: 9 % (6/65) of the CTM group, and 3 % (2/67) of the ACA group (p = 0.30). Side effects were significantly more frequent for the ACA group (34/95, 36 %) than for the CTM group (13/95, 14 %) (p = 0.001). The authors conclude thar the efficacy of a ten-day course of cefotiam hexetil 200 mg in two daily intakes is not different from that a ten-day course of ACA 1 g in two daily intakes, for adults with chronic otitis media. Nevertheless, side effects are significantly less frequent with cefotiamhexetil.
