Abstract

Contribution to diagnosis: Magnetic resonance imaging (MRI) is the first exploration to be ordered in case of suspected multiple sclerosis. MRI of the head or spine can visualize the lesions as spontaneously intense signals. These lesions are however non-specific. Intravenous gadolinium injection can demonstrate different stages of development. Magnetic resonance spectroscopy: MRS reveals biochemical abnormalities within the white matter which can appear normal on MRI. Well-identifiedpathogenic sequence: Five stages have been recognized based on MRI and MRS findings: blood-brain barrier permeability, development of active inflammatory lesions, demyelinisation, gliosis, and finally axon loss. Clinical trials: MRI is particularly adapted to repeated monitoringof the efficacy of new therapeutics. (C) 1997, Masson, Paris.
