Abstract

Tumors of the pancreas may be of exocrine or endocrine origin. When exocrine pancreatic cancer becomes symptomatic it has generally alreadyspread, the most common symptoms being anorexia, weight loss and pain. Endocrine tumors rarely produce these symptoms, except glucagonomas which often are accompanied by weight loss. Additionally, pain is seldom seen , at least in the early stages, in patients with endocrine tumors. Diabetes and glucose intolerance is much more frequent in exocrine pancreatic cancer (70-80% of patients) than earlier anticipated and it is part of the symptomatology of certain endocrine tumors. Attemptsat earlier diagnosis of exocrine pancreatic cancer have hitherto beendisappointing. It is suggested that early tumor markers for exocrine cancer may be identified when the metabolic disturbances behind the marked weight loss and the glucose intolerance become clarified. Possibly serum measurement of peptides such as islet amyloid polypeptide or enterostatin may turn out to be of value as are the specific peptides in the diagnosis of endocrine tumors. Imaging tests such as ultrasonography and computerized tomography demonstrate pancreatic lesions with adiameter more than 1.5-2.0 cm and arteriography is of value in highlyvascularized endocrine tumors. The differential diagnosis between demonstrable endocrine and exocrine pancreatic tumors is generally easy to make. However, to distinguish between nonfunctioning endocrine tumors and cystic adenocarcinomas of the gland must often await histological and/or histochemical examination.
