Abstract

After the introduction of a new drug on the market, there is little formal control and evaluation. Clinicians have to experiment more or less on their own to learn how to use new drugs. The value of ''new generation'' antidepressants is extensively discussed. We studied what happened in our usual clinical setting when two new drugs were introduced. The design was simple. The only criterion for inclusion was intention to treat with one of the new drugs. We requested all such intentionsreported to us. At the end of 16 months we extracted data from the medical records. We found 168 intentions to treat, 53 with fluvoxamine and 115 with paroxetine. Complicated cases dominated in our material. Few of these patients would fulfil criteria for inclusion in a formal study. Fluvoxamine was introduced first and dominated in the early phase of the study; then there was a shift in favour of paroxetine. Efficacy was considered good or moderately good in only 34% (fluvoxamine) to54% (paroxetine). The most important factor contributing to this modest result was withdrawal in 42%, mainly due to side effects and insufficient efficacy. Withdrawal rated higher and results were poorer with the first drug. We discuss the possibility of some ''first drug effect'', containing factors such as lack of experience with the new drug orfamily of drugs. The study has stimulated colleagues to share experiences and knowledge of new drugs, which adds a didactic value.
