Abstract

Major depression with psychotic features (PMD) represents a distinct and severe subtype of depression. This article reviews characteristicsthat distinguish PMD from nonpsychotic major depression. Emphasis is placed on increased activity of the hypothalamic-pituitary-adrenal axis and dopamine systems in PMD. Interactions between the two systems are reviewed as a means for understanding the biological basis for psychotic thinking in depressed patients. Strategies for addressing key research questions in this area are discussed.
