Abstract

Diffusion-weighted imaging (DWI) and perfusion Imaging (PI), new magnetic resonance imaging (MRI) tools, are useful for stroke diagnosis and the in vivo evaluation of pharmacological intervention and are currently transitioning from experimental studies to clinical application. The apparent diffusion coefficient (ADC) of water quickly drops after ischemia, slowly reverts to normal over 24-48 h in animals and then further increases to above normal levels. DWI, based on water molecular diffusion, can rapidly identify and quantify ischemic changes within several minutes after onset of ischemia in animal stroke models. With ADC mapping, reversible ischemic alterations might be differentiated from irreversible ones, providing physicians with very important information to initiate drug therapy for ischemic stroke. PI, usually done with the bolus injection of contrast, can be used to delineate brain perfusion and map relative cerebral blood volume (rCBV). It has the ability to both qualitatively and quantitatively demonstrate early ischemiaand to potentially identify the ischemic penumbra. The development ofultrafast MRI provides the ability to do DWI and PI in ischemic stroke patients. DWI and PI in ischemic stroke patients offer earlier definite diagnosis, more accurate localization and a better index in predicting the clinical outcome of acute ischemic stroke than conventional MRI and they can distinguish between acute and chronic stroke. The combined use of DWI and PI provides evaluation for ischemic stroke from regional anatomical to pathophysiological changes.
