Abstract

Extrapyramidal syndromes (EPS) represent that group of adverse effects most commonly associated with conventional antipsychotic drugs. Aggressive management of EPS is essential, as they can be subjectively distressing, potentially life-threatening and, at limes, irreversible. They can obscure diagnostic issues, as well as contribute to noncompliance. Pharmacological intervention is routinely employed and consists primarily of antiparkinsonian agents, except in akathisia, where these compounds have limited efficacy. The choice of an appropriate treatmentmay be guided by a variety of factors, including the particular type of EPS, individual patient characteristics, and specific drug featuresof the medications being considered, e.g. availability, formulation, pharmacokinetics, pharmacodynamics, and adverse effect profile. While the incidence of EPS is reduced with the newer 'atypical' antipsychotics, such as clozapine and risperidone, EPS is not eliminated and, at present, effective use of antipsychotics continues to require proficiency in the management of these troublesome adverse effects.
