Abstract

This review discusses the control of ion exchanges and paracellular channels, the influence of the enteric nervous system on salt and watertransport, the role of an endogenous secretagogue, and the optimization of rehydration therapy in diarrheal states. A synergistic mechanismof action between Na+/H+ exchanges of the apical and basolateral membranes is postulated and glutamine is found to activate this exchange at a cellular level, Nitric oxide is implicated in the energy-dependent, stress-sensitive maintenance of paracellular channel integrity. Coordination of both absorptive and secretory states is mediated by the recruitment of a sophisticated local nervous system, Roles for the ligand that mimics a heat-stable enterotoxin are proposed and, in the wake of a sobering report on the recent cholera epidemic in Coma, an important study confirms the merits of hypotonic oral rehydration solution.
